Holistic Home Health Care, Inc. accepts payments through private funding and many forms of insurance.

Private Funding:
Private funding is defined by Holistic Home Health Care, Inc. as payment that is delivered to the business through private individuals such as the client receiving care or financial powers of attorney. Check and Credit Card are accepted.

Insurance:
Accepted insurance is defined by Holistic Home Health Care, Inc. as payment that is delivered to the business through organizations and not by private individuals. Our agency works with the following insurance agencies:

iCare: Independent Care Health Plan is a managed care organization providing insurance benefits for people with disabilities and low-income Wisconsin residents.

Medicare: This is a federal health insurance program for individuals who are age 65 and above. If you are receiving Social Security Disability Insurance, you can also qualify for Medicare even if you are under 65 years old.

My Choice Family Care: My Choice Family Care is a managed care organization that serves nine counties in Wisconsin. They provide insurance benefits for people who have long-term care service needs, meet financial eligibility requirements, and be a frail elder or an adult with a disability.

IRIS: The IRIS program is a Medicaid Home And Community-Based Services waiver for self-directed long-term supports. IRIS is available to Wisconsin residents who are determined to be financially eligible and are in need of a determined level of care.

Community Care: Community Care is a nonprofit organization that helps eligible adults with long-term care needs live as independently as possible. Community Care serves Wisconsin residents in 14 counties.

ContinuUs: ContinuUs is a managed care organization that provides insurance benefits for eligible Wisconsin adults in eight counties. Individuals must prove eligibility through financial and health requirements.

Title 19: Title 19 is a Medicaid plan for the elderly, blind or disabled who meet varying levels of eligibility. Eligibility is based on age, disability, income, and citizenship.

Worker’s Compensation: Worker’s Compensation provides wage replacement and medical benefits to covered employees injured on the job. If injured on the job, please contact your employer to discuss your coverage.

Private Insurance: Various forms of private insurances may be accepted. The most common form that will cover the costs of health services will be a long-term insurance provider. This type of insurance can be used when other forms of health insurance, Medicare, or Medicaid do not cover costs.
